A printed version of this form is available upon request in lieu of the online form. The online version, or a paper substitute, must be completed and submitted before the child can attend Beach Club. A separate form must be completed annually for each child in your family who will attend Beach Club.

From time to time KiDs Beach Club® or the partnering church may take photographs/video to capture the fun kids have at Beach Club.

Email addresses provided on this form will automatically be added to the KiDs Beach Club® mailing list and be provided to the partnering church. We promise to only send a reasonable amount of communication each month. You may unsubscribe at any time, beginning with our first contact.

T-shirts are provided at the discretion of the partnering church.
